Other than for routine care or tests related to HIV, in the past 12 months have you been advised by a licensed medical professional to see a medical specialist, have an in-patient surgery, biopsy, hospitalization, diagnostic test or procedure, which has not been completed or the results are unknown?
The answer must be “no” to qualify.
In the past five years, have you been convicted of a felony or are you currently on parole or probation?
The answer must be “no” to qualify.
Are you receiving assistance or supervision with eating, bathing, dressing, walking, toileting, or getting up out of a chair?
The answer must be “no” to qualify.
Have you been diagnosed with a terminal illness with a life expectancy of 24 months or less?
Answer must be “no” to qualify.
Have you ever been diagnosed with a serious medical condition?
If the answer is “yes,” you may refer to Appendix A of the Application Process Guide for further qualifications. A “yes” answer to this does not automatically disqualify an applicant.
Do you have either U.S. citizenship or a U.S. permanent resident card (green card)?
The answer must be “yes” to qualify.
Ages 71+ only – have you seen a medical professional in the past 12 months?
The answer must be “yes” to qualify.
After your client has been approved for XL Heritage, your client will need to sign the electronic application. After your client's premium or 1035 Exchange documentation is submitted, the process is complete. It's important to note that your client's life insurance is not active until Midland National receives premium or the 1035 Exchange payment.

1. Although XL Heritage is filed as a Flexible Premium Universal Life and allows additional premiums, the product is designed for a single premium payment. There are limited benefits to adding premiums after policy issue – for example, the Account Value will increase with additional premium payments, but the guaranteed death benefit and the Return of Premium Value will NOT. Therefore, clients should carefully consider whether additional premiums are paid.
